Through them I've seen the same pattern over and over: a patient pays $1,500-$2,500 out of pocket for crowns or a root canal, then files an out-of-network reimbursement claim with their PPO, and ends up walking away with nothing or pennies on the dollar. Not because the work wasn't covered — but because the claim wasn't filed in the format the insurer wanted. One wrong CDT code (D2750 vs D2740 — porcelain-fused-to-metal vs porcelain crown), a missing field on the factura, an exchange-rate dispute.
